A Biller or Funding Authorisation Form must be completed when a customer will be placing their own orders, but an Organisation will be responsible for paying the invoices.
This form MUST be completed and submitted to Independence Australia by the Organisation paying the invoices. Forms submitted by the customer directly will not be accepted
In addition, the funding Organisation is responsible for advising Independence Australia in writing as soon as this arrangement ceases or changes
